How we handle your health information.

SlimFit Wellness Clinic & Medical Aesthetics is required by law to maintain the privacy of your protected health information, to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

How we may use and disclose your health information

For treatment

We use your health information to provide care. For example, a provider reviewing your health history, lab work and current medications to determine whether a treatment is appropriate for you, or sharing that information with another provider involved in your care.

For payment

We use and disclose your health information to bill and collect payment for the care you receive, including confirming eligibility and coverage where a plan is involved.

For health care operations

We use your health information to run the practice: quality review, provider evaluation, training, business planning, and administrative functions.

Appointment reminders and follow-up

We may contact you to remind you of an appointment, to follow up after treatment, or to tell you about treatment alternatives or health-related benefits and services that may interest you. If you prefer we contact you a particular way or at a particular number or address, tell us and we will accommodate reasonable requests.

Others involved in your care

We may share information with a family member, friend or other person you identify as involved in your care or payment for your care, limited to what is relevant. If you are present, we will give you the chance to object first.

As required or permitted by law

We may use or disclose your health information without your authorization where the law requires or permits it, including: public health activities; reporting suspected abuse, neglect or domestic violence; health oversight activities; judicial and administrative proceedings; law enforcement purposes; coroners, medical examiners and funeral directors; organ donation; research under approved protocols; to avert a serious threat to health or safety; specialized government functions; and workers' compensation.

Uses that always require your written authorization

We will not use or disclose your health information for the following without your written authorization:

• Marketing. Most uses and disclosures of your health information for marketing purposes.
• Sale of your information. We do not sell your protected health information.
• Psychotherapy notes, where any are maintained.
• Any other use or disclosure not described in this notice.

If you give us written authorization, you may revoke it in writing at any time. A revocation does not undo anything we already did in reliance on it.

Your rights

• Inspect and copy. You may inspect and get a copy of your health and billing records. We will provide an electronic copy if you ask and we maintain it electronically. We may charge a reasonable, cost-based fee.
• Request an amendment. If you believe information in your record is incorrect or incomplete, you may ask us to amend it. We may deny the request in certain cases, and you may file a statement of disagreement.
• An accounting of disclosures. You may request a list of certain disclosures we made of your health information.
• Request restrictions. You may ask us to limit how we use or disclose your information. We are not required to agree, with one exception below.
• Restrict disclosure to a health plan. If you pay for a service in full out of pocket, you may tell us not to share information about that service with your health plan, and we must honor that request.
• Confidential communications. You may ask us to contact you at a specific address or by a specific method.
• A paper copy of this notice, even if you agreed to receive it electronically.
• Notice of a breach of unsecured protected health information.
• Choose someone to act for you. A personal representative with legal authority may exercise these rights on your behalf.

Our responsibilities

We are required by law to maintain the privacy and security of your protected health information, to notify you promptly if a breach occurs that may have compromised it, and to follow the terms of the notice currently in effect. We reserve the right to change this notice and to make the revised notice effective for information we already hold as well as information we receive in the future. The current notice will always be posted on this page and available at both clinics.

How to exercise a right or make a complaint

We will not retaliate against you for filing a complaint.
