SlimFit Wellness Clinic & Medical Aesthetics ("SlimFit," "we," "us") operates clinics in Sarasota and Venice, Florida. This policy explains what we collect, how we use it, and the choices you have.

Two different things, kept separate on purpose. Your medical information as a patient is protected health information under HIPAA. It is never used for advertising and is never sold or shared for advertising. Separately, this website uses ordinary advertising and measurement tools that do pass limited identifiers to third parties. Both are described below, and you can turn the second one off at any time on Your Privacy Choices.

Your health information

Information you give us as a patient, including your health history, treatment records, lab work and anything you discuss with a provider, is protected health information. It is handled under HIPAA and under our Notice of Privacy Practices, which governs how we may use and disclose it and what rights you have over it. A paper copy is available at either clinic on request.

We do not use protected health information for advertising. We do not sell it, and we do not share it with advertising platforms.

Information the website collects

Separately from your patient record, when you visit this website we collect:

  • Identifiers. Name, email address and phone number if you submit them, plus IP address and cookie or device identifiers.
  • Internet and network activity. Pages viewed, links and buttons clicked, time on page, referring site, and how you interact with our ads.
  • Approximate location. Derived from your IP address. We do not collect precise geolocation from the website.
  • Commercial information. Which services you looked at or asked about.
  • Inferences drawn from the above, used to measure which ads and pages bring people to our clinics.

Advertising and analytics on this website

We use the following. Two of them pass identifiers to companies outside our control, which California law treats as sharing personal information for cross-context behavioral advertising:

  • Google Ads conversion tracking and remarketing. Tells us which ads lead to appointments and allows ads to be shown to people who have visited this site. This involves sharing identifiers with Google.
  • A social advertising pixel. Measures which ads bring people to the site. This involves sharing identifiers with the platform.
  • A session and behavior analytics tool. Records how visitors move through pages so we can find what is confusing or broken.
  • Privacy-focused site analytics. Counts visits and page views without cookies and without building a profile of you.

We do not sell your personal information for money, and none of these tools receive your patient records. They do receive website activity, which is why the opt-out below exists and why we honor it automatically when your browser asks.

How we use your information

  • Schedule and deliver care, follow up on treatment, and send appointment reminders
  • Answer questions and respond to inquiries
  • Measure which advertising and which pages actually bring people to our clinics
  • Maintain, secure and improve the website
  • Meet legal, medical recordkeeping and tax obligations

Your privacy rights

Depending on where you live, you may have the right to:

  • Know what personal information we have collected, where it came from, why, and who we disclosed it to
  • Delete personal information we hold about you
  • Correct inaccurate personal information
  • Opt out of the sharing of your personal information for targeted advertising
  • Limit our use of sensitive personal information, which includes health information, to what is necessary to provide the services you asked for
  • Not be discriminated against for exercising any of these rights. We will not deny you care, charge you a different price, or give you a lower level of service because you exercised a privacy right.

Rights over your medical record itself, including access, amendment and an accounting of disclosures, come from HIPAA and are described in our Notice of Privacy Practices.

How to exercise your rights

Either method works for any request:

To opt out of advertising sharing without contacting us, use Your Privacy Choices. We also honor the Global Privacy Control signal automatically. If your browser or extension sends GPC, we treat it as an opt-out request with no further action needed from you.

We verify requests by matching what you give us against what we already hold, and for medical records we follow the verification process in our Notice of Privacy Practices. An authorized agent may submit a request on your behalf with written permission signed by you, and we may still contact you directly to confirm.

How long we keep information

  • Medical records: as required by Florida law and our medical recordkeeping obligations
  • Inquiries that did not become appointments: up to 24 months, then deleted
  • Website analytics and advertising data: up to 26 months, on the platforms' standard retention
  • Billing and tax records: as long as tax law requires

Service providers

We use third-party providers for scheduling, email, payments, website hosting, advertising and analytics. They may only use your information to perform services for us, and providers who handle protected health information do so under a business associate agreement.

Security

We use reasonable administrative, physical and technical safeguards to protect the information we hold. No method of transmission or storage is completely secure, and we cannot guarantee absolute security.

Children

This website is not directed to children, and we do not knowingly collect information from anyone under 18 through it.

Changes to this policy

If we change this policy we will update the date at the top of this page. Material changes will be noted on the site.
